1) The Radford University Ice Hockey Club cannot give athletic scholarships or anyway assist in the financing of a players dues or education as stated in ACHA guidelines. Visit www.radford.edu for scholarship information.

2) Before you fill out this form you must plan on attending RU with a minimum of 12 credit hours during the fall of 2003.*

3) Players must still meet with an officer at Club Fair.
